Questions about accessing protective vaccines are swirling.<\/p>\n<p>Nearly five years since the first U.S. patient got vaccinated to help stem what was then a raging coronavirus pandemic, doctors, patients and pharmacists are navigating a radically different public health landscape.<\/p>\n<p>Data point No. 1: The U.S. Food and Drug Administration on Aug. 27 <a href=\"https:\/\/mvau.lt\/media\/24142bfe-d052-4133-a6eb-bc4991893721\" target=\"_blank\" rel=\"noopener nofollow\"><u>announced<\/u><\/a> it had approved some COVID-19 vaccines \u2014 but for far fewer people than in years past.<\/p>\n<p><strong>READ MORE:<\/strong> <a href=\"https:\/\/www.wlrn.org\/health\/2025-08-13\/florida-covid-cases-spike-right-before-the-start-of-schools\" rel=\"nofollow noopener\" target=\"_blank\">Florida COVID cases spike right before the start of schools<\/a><\/p>\n<p>Data point No. 2: If you tried to schedule a COVID-19 vaccine this week, your pharmacy\u2019s online scheduler might have told you it isn\u2019t available yet.<\/p>\n<p>Data point No. 3: Leading medical organizations disagree with the government\u2019s latest COVID-19 vaccine guidance.<\/p>\n<p>If all that leaves you wondering about your ability to get a COVID-19 shot, you\u2019re not alone. It\u2019s confusing.<\/p>\n<p>We sought answers.<\/p>\n<p><strong>Q: Who can get a COVID-19 vaccine?<\/strong><\/p>\n<p>First, what we do know: The FDA <a href=\"https:\/\/www.pfizer.com\/news\/press-release\/press-release-detail\/pfizer-and-biontechs-comirnatyr-receives-us-fda-approval\" target=\"_blank\" rel=\"noopener nofollow\"><u>approved<\/u><\/a> <a href=\"https:\/\/ir.novavax.com\/press-releases\/2025-08-27-Novavaxs-Nuvaxovid-TM-2025-2026-Formula-COVID-19-Vaccine-Approved-in-the-U-S\" target=\"_blank\" rel=\"noopener nofollow\"><u>the<\/u><\/a> 2025-26 vaccines for anyone age 65 and older and any person 6 months and older who has at least one underlying health condition that increases their risk of severe COVID-19 infection.<\/p>\n<p>So, people in those groups should be able to schedule vaccinations as soon as health care providers who are authorized to administer the vaccine receive it \u2014 likely in the next few weeks.<\/p>\n<p><strong>Q: I don\u2019t fit into those categories. Am I banned from getting a COVID-19 vaccine?<\/strong><\/p>\n<p>Not \u201cbanned,\u201d per se. But it may require more legwork on your end. You\u2019ll need to consult your doctor, who can legally prescribe a COVID-19 vaccine for you even if you don\u2019t fall within the categories the FDA specified.<\/p>\n<p>\u201cPhysicians can prescribe medications and vaccines that are beyond the label, beyond the license,\u201d said Dr. William Schaffner, Vanderbilt University Medical Center professor of infectious diseases.<\/p>\n<p>That\u2019s true for adults and children \u2014 and the practice of prescribing medications and vaccines for \u201coff-label\u201d use is fairly common in pediatrics, he said.<\/p>\n<p><strong>Q: OK, so will insurance still cover COVID-19 vaccinations under these new rules?<\/strong><\/p>\n<p>If you\u2019re in one of the two FDA-approved categories the outlook is probably better than if you\u2019re not.<\/p>\n<p>Federal law requires that most health insurance plans <a href=\"https:\/\/www.nbcnews.com\/health\/health-news\/-fall-covid-shots-may-come-copays-no-coverage-rcna226160\" target=\"_blank\" rel=\"noopener nofollow\"><u>fully cover<\/u><\/a> vaccines recommended by the U.S. Centers for Disease Control and Prevention and its vaccine advisory committee.<\/p>\n<p>Here\u2019s the rub: Because the CDC hasn\u2019t yet adopted 2025-26 COVID-19 vaccine recommendations, whether the shots will be covered by insurance is a gray area.<\/p>\n<p>The CDC\u2019s panel of experts, the Advisory Committee on Immunization Practices, had planned to vote on updated COVID-19 vaccine recommendations at its June meeting but did not. And it\u2019s unclear when or if the ACIP will vote on the recommendations.<\/p>\n<p>But there could be another avenue for people to get insurance-covered COVID-19 vaccines. The CDC\u2019s vaccine recommendations typically include a provision for what\u2019s known as \u201cshared clinical decision making,\u201d Schaffner said.<\/p>\n<p>That means, for example, \u201cIf you were 52 years old and otherwise healthy, but you nonetheless wanted to get the vaccine, you could discuss that with your doctor \u2014 shared clinical decision making \u2014 and you could receive the vaccine, and insurance companies would honor that,\u201d he said.<\/p>\n<p>That provision could be important for healthy people who want to avoid COVID-19 infection because they are close contacts with or care for people with high risk conditions.<\/p>\n<p>But again, without CDC recommendations, \u201cWe don\u2019t know if that provision is still there,\u201d Schaffner said.<\/p>\n<p>To confirm whether your insurance will cover your COVID-19 shot, Schaffner recommends talking to your doctor and, potentially, your insurance provider. But first, give it a little more time: Healthcare providers themselves are still working out the details.<\/p>\n<p><strong>Q: If my insurance doesn\u2019t cover vaccination, how much might it cost?<\/strong><\/p>\n<p>Vaccine manufacturers report that COVID-19 vaccines cost about $142, according to the <a href=\"https:\/\/www.cdc.gov\/vaccines-for-children\/php\/awardees\/current-cdc-vaccine-price-list.html\" target=\"_blank\" rel=\"noopener nofollow\"><u>CDC\u2019s price lists<\/u><\/a>. It\u2019s unclear whether that would be the out-of-pocket cost for patients receiving a COVID-19 vaccine not covered by insurance.<\/p>\n<p><strong>Q: Are COVID-19 vaccines still recommended during pregnancy? And if I\u2019m pregnant and want one, what can I do?<\/strong><\/p>\n<p>Health and Human Services Secretary Robert F. Kennedy Jr. in May announced he\u2019d unilaterally removed COVID-19 vaccines from the recommended <a href=\"https:\/\/x.com\/SecKennedy\/status\/1927368440811008138\" target=\"_blank\" rel=\"noopener nofollow\"><u>immunization schedule<\/u><\/a> for healthy children and pregnant women. The health-focused news outlet <a href=\"https:\/\/www.statnews.com\/2025\/05\/27\/covid-shots-pregnant-women-children-recommendation-change-hhs-secretary-kennedy\/\" target=\"_blank\" rel=\"noopener nofollow\"><u>STAT reported<\/u><\/a> that no one from the CDC or ACIP was consulted before Kennedy announced this change. Vaccine experts called <a href=\"https:\/\/www.pbs.org\/newshour\/show\/why-public-health-groups-are-concerned-about-changes-to-covid-vaccine-recommendations\" target=\"_blank\" rel=\"noopener nofollow\"><u>the move concerning and unprecedented<\/u><\/a>.<\/p>\n<p>Removing the vaccines from the immunization schedule could limit vaccine access by <a href=\"https:\/\/www.pbs.org\/newshour\/health\/who-is-eligible-for-a-covid-shot-what-to-know-about-the-fdas-latest-changes\" target=\"_blank\" rel=\"noopener nofollow\"><u>reducing<\/u><\/a> insurance companies\u2019 <a href=\"https:\/\/www.npr.org\/sections\/shots-health-news\/2025\/05\/27\/nx-s1-5413179\/covid-vaccine-children-pregnant-rfk-cdc\" target=\"_blank\" rel=\"noopener nofollow\"><u>coverage<\/u><\/a> of the shot. But again, by consulting with your doctor, you might still be able to access it.\u00a0Some pharmacies also might be able to provide it, because the CDC lists pregnancy as a factor that increases risk of severe COVID-19 infection.<\/p>\n<p><strong>Q: Is there anyone who should not get the COVID-19 shot?<\/strong><\/p>\n<p>Broad health guidance always comes with exceptions. For example, people who have had \u201ca very severe adverse reaction to a previous COVID-19 vaccine\u201d should not get this season\u2019s version, Schaffner said. You should discuss your health situation with your physician for personalized guidance.<\/p>\n<p><strong>Q: Why is the outlook for COVID-19 vaccines so different this year?<\/strong><\/p>\n<p>In short, Kennedy has long been broadly critical of vaccinations and, now that he\u2019s in charge of the nation\u2019s health care policy, has taken actions to overhaul their regulation and distribution.<\/p>\n<p>Typically, the CDC recommends vaccines based on guidance from the ACIP. ACIP\u2019s recommendations become CDC policy if they\u2019re <a href=\"https:\/\/www.cdc.gov\/acip\/vaccine-recommendations\/index.html\" target=\"_blank\" rel=\"noopener nofollow\"><u>adopted<\/u><\/a> by the CDC director. This year, Kennedy fired all 17 members of ACIP and <a href=\"https:\/\/www.politifact.com\/article\/2025\/jun\/18\/rfk-jr-fires-vaccine-panel-replacements\/\" target=\"_blank\" rel=\"noopener nofollow\"><u>replaced them<\/u><\/a> with new members, many of whom have expressed antivaccine views. CDC Director Susan Monarez <a href=\"https:\/\/www.washingtonpost.com\/health\/2025\/08\/27\/susan-monarez-cdc-director-ousted\/\" target=\"_blank\" rel=\"noopener nofollow\"><u>was fired<\/u><\/a> Aug. 27 amid a <a href=\"https:\/\/www.cidrap.umn.edu\/public-health\/cdc-director-fired-after-she-refused-rubber-stamp-kennedys-vaccine-directives\" target=\"_blank\" rel=\"noopener nofollow\"><u>dispute with Kennedy<\/u><\/a> over vaccine policy.<\/p>\n<p>In previous years, the CDC <a href=\"https:\/\/apnews.com\/article\/vaccines-covid-shots-fda-kennedy-pfizer-children-9ce19908ae7afb354732aa162fd28532\" target=\"_blank\" rel=\"noopener nofollow\"><u>recommended<\/u><\/a> <a href=\"https:\/\/www.cdc.gov\/mmwr\/volumes\/73\/wr\/mm7316a4.htm\" target=\"_blank\" rel=\"noopener nofollow\"><u>annual<\/u><\/a> COVID-19 vaccines for <a href=\"https:\/\/www.cdc.gov\/mmwr\/volumes\/73\/wr\/mm7349a2.htm\" target=\"_blank\" rel=\"noopener nofollow\"><u>everyone<\/u><\/a> ages 6 months and older. The committee still hasn\u2019t issued COVID-19 vaccine recommendations for this year.<\/p>\n<p><strong>Q: Aren\u2019t there some medical organizations that recommend COVID-19 vaccinations for most people, though?<\/strong><\/p>\n<p>Yes. In recent months, professional organizations have issued their own recommendations that contradict new messaging from the federal government.<\/p>\n<p>The American Academy of Pediatrics <a href=\"https:\/\/publications.aap.org\/redbook\/resources\/15585?_gl=1*1i2jy21*_ga*MTc4MTIyODEyNi4xNzU0NjgzMjg2*_ga_FD9D3XZVQQ*czE3NTYzOTk2NDIkbzIyJGcxJHQxNzU2Mzk5NzU3JGo2MCRsMCRoMA..*_gcl_au*MTI5NDE2Mzc3LjE3NTQ2ODYyODk.*_ga_GMZCQS1K47*czE3NTYzOTk2NjYkbzEyJGcxJHQxNzU2Mzk5NzU3JGo2MCRsMCRoMA..\" target=\"_blank\" rel=\"noopener nofollow\"><u>recommends<\/u><\/a> routine COVID-19 vaccines for all children ages 6 months through 23 months and for kids ages 2 to 18 in some situations including if they\u2019re at high risk of COVID-19, have never been vaccinated against it or live in a household with people who are high risk.<\/p>\n<p>And the American College of Obstetricians and Gynecologists <a href=\"https:\/\/www.acog.org\/clinical\/clinical-guidance\/practice-advisory\/articles\/2020\/12\/covid-19-vaccination-considerations-for-obstetric-gynecologic-care\" target=\"_blank\" rel=\"noopener nofollow\"><u>recommends<\/u><\/a> that anyone pregnant or lactating receive updated or \u201cbooster\u201d COVID-19 vaccines.<\/p>\n<p>Dr. Tina Tan, president of the Infectious Disease Society of America, <a href=\"https:\/\/www.idsociety.org\/news--publications-new\/articles\/2025\/fdas-narrow-covid-19-vaccine-label-ignores-science-and-puts-millions-of-lives-at-risk\/\" target=\"_blank\" rel=\"noopener nofollow\"><u>said<\/u><\/a> the FDA\u2019s decision to limit the COVID-19 vaccine\u2019s approval \u201ccontradicts the evidence base, severely undermines trust in science-driven policy and dangerously limits vaccine access.\u201d<\/p>\n<p><strong>Q: What qualifies as an underlying health condition that puts someone at higher risk?<\/strong><\/p>\n<p>The FDA and HHS did not point us to a list of health conditions that meet the high risk requirements.<\/p>\n<p>The CDC\u2019s website catalogs <a href=\"https:\/\/www.cdc.gov\/covid\/risk-factors\/index.html\" target=\"_blank\" rel=\"noopener nofollow\"><u>a number of<\/u><\/a> <a href=\"https:\/\/www.cdc.gov\/covid\/hcp\/clinical-care\/underlying-conditions.html\" target=\"_blank\" rel=\"noopener nofollow\"><u>underlying conditions<\/u><\/a> that it said demonstrate \u201ca conclusive increase in risk for at least one severe COVID-19 outcome.\u201d They included: chronic lung diseases, cancer, certain chronic kidney and liver diseases, diabetes, some disabilities, heart conditions, HIV, physical inactivity, primary immunodeficiencies and some mental health conditions.<\/p>\n<p><strong>Q: But I clicked on those lists and both say \u201cpregnancy and recent pregnancy\u201d are risk factors. How do I square that with the latest changes?<\/strong><\/p>\n<p>You\u2019re right. These lists are inconsistent with the messaging coming from top HHS officials. Again, the American College of Obstetricians and Gynecologists continues to <a href=\"https:\/\/www.acog.org\/clinical\/clinical-guidance\/practice-advisory\/articles\/2020\/12\/covid-19-vaccination-considerations-for-obstetric-gynecologic-care\" target=\"_blank\" rel=\"noopener nofollow\"><u>recommend<\/u><\/a> COVID-19 vaccines for people who are pregnant and lactating. Talk to your doctor.<\/p>\n<p><strong>Q: Is this season\u2019s COVID-19 vaccine formula different from last season\u2019s?<\/strong><\/p>\n<p>Yes. After the FDA passed its recommendations to vaccine manufacturers <a href=\"https:\/\/www.fda.gov\/vaccines-blood-biologics\/industry-biologics\/covid-19-vaccines-2025-2026-formula-use-united-states-beginning-fall-2025\" target=\"_blank\" rel=\"noopener nofollow\"><u>in May<\/u><\/a>, the COVID-19 vaccines were updated to target the viral strain expected to circulate this year. The changes align with the World Health Organization\u2019s <a href=\"https:\/\/www.who.int\/news\/item\/15-05-2025-statement-on-the-antigen-composition-of-covid-19-vaccines\" target=\"_blank\" rel=\"noopener nofollow\"><u>recommendations<\/u><\/a>.<\/p>\n<p><strong>Q: When will the updated COVID-19 shots be available?<\/strong><\/p>\n<p>Since FDA\u2019s approval, updated vaccines are set to start shipping immediately and might even be available now in some pharmacies. Pfizer <a href=\"https:\/\/www.pfizer.com\/news\/press-release\/press-release-detail\/pfizer-and-biontechs-comirnatyr-receives-us-fda-approval\" target=\"_blank\" rel=\"noopener nofollow\"><u>said<\/u><\/a> it was shipping immediately and would be available across the U.S. \u201cin the coming days.\u201d Sanofi, the company distributing the Novavax non-mRNA vaccines, said its vaccine should be available \u201cin the early fall.\u201d<\/p>\n<p>But what that means for how quickly you can access it could depend on where you live, your age and your health. (See next question!)<\/p>\n<p>Q: Where can I get the updated COVID-19 shots?<br \/>We knew you were going to ask. And we wish we had a straightforward answer.<\/p>\n<p>If you typically get your COVID-19 shots at a local pharmacy, it might not be that easy this year.<\/p>\n<p>As of Aug. 29, the scheduling apps for Walgreens and CVS notified patients in some locations that they could not schedule a COVID-19 vaccine appointment because of state restrictions, inventory or the need for a prescription.<\/p>\n<p>In 18 states and Washington, D.C., pharmacists\u2019 authority to administer vaccines is linked to the CDC\u2019s recommendations, said Brigid Groves, American Pharmacists Association\u2019s vice president of professional affairs. The states are: Colorado, Connecticut, Georgia, Iowa, Kentucky, Maine, Maryland, Massachusetts, Montana, Nevada, New Jersey, New Mexico, North Carolina, Oregon, Pennsylvania, South Carolina, Virginia and West Virginia.<\/p>\n<p>That means even though the FDA has issued its approval, in those 19 places, pharmacists cannot administer it because it isn\u2019t on the CDC immunization schedule yet, Groves said.<\/p>\n<p>The American Pharmacists Association has asked those states\u2019 governors to issue executive orders granting pharmacists broader authority to administer vaccines.<\/p>\n<p>In the other 32 states, with some exceptions, it\u2019s possible that a pharmacy can administer the updated COVID-19 shots for patients who fall within the FDA-approved categories of recipients.<\/p>\n<p>If you are getting it \u201coff label,\u201d however, because you don\u2019t have one of those underlying conditions, you might need to get it straight from your prescribing doctor.<\/p>\n<p>One caveat: If the ACIP votes on recommendations for COVID-19 vaccines that includes giving them to healthy people through the \u201cshared clinical decision making\u201d process, Groves said pharmacists would be able to vaccinate almost anyone. That\u2019s because pharmacists are considered clinicians who can conduct that shared decision making.<\/p>\n<p><strong>Q: When might the ACIP vote on COVID-19 vaccine recommendations?<\/strong><\/p>\n<p>The committee has a Sept 18-19 meeting scheduled, according to <a href=\"https:\/\/www.cdc.gov\/acip\/meetings\/index.html\" target=\"_blank\" rel=\"noopener nofollow\"><u>the CDC\u2019s website<\/u><\/a>. A <a href=\"https:\/\/www.cdc.gov\/acip\/meetings\/agendas.html\" target=\"_blank\" rel=\"noopener nofollow\"><u>meeting agenda<\/u><\/a> hasn\u2019t been published yet.<\/p>\n<p><strong>Q: How will a pharmacy know if I have an underlying condition?<\/strong><\/p>\n<p>Typically, pharmacies ask patients to self-attest whether they have an underlying condition, Groves said. For example, a person who is under age 65 but has severe asthma would self-attest to that when making a vaccine appointment. So far, all signs point to that self-attestation still being the case.<\/p>\n<p><strong>Q: When is the best time to get vaccinated?<\/strong><\/p>\n<p>During <a href=\"https:\/\/www.lung.org\/blog\/respiratory-virus-season-prevention\" target=\"_blank\" rel=\"noopener nofollow\"><u>fall and winter<\/u><\/a> months, when COVID-19 infections are typically <a href=\"https:\/\/health.clevelandclinic.org\/respiratory-season\" target=\"_blank\" rel=\"noopener nofollow\"><u>expected<\/u><\/a> to spike alongside other respiratory infections.<\/p>\n<p><strong>Q: What do the administration\u2019s vaccine changes mean for future COVID-19 vaccines?<\/strong><\/p>\n<p>Kennedy recently <a href=\"https:\/\/apnews.com\/article\/kennedy-vaccines-mrna-pfizer-moderna-1fb5b9436f2957075064c18a6cbbe3c9\" target=\"_blank\" rel=\"noopener nofollow\"><u>canceled $500 million in funding<\/u><\/a> for mRNA vaccine development. Two of the most effective COVID-19 vaccines are mRNA. Some companies had been <a href=\"https:\/\/apnews.com\/article\/flu-covid-combination-shot-moderna-vaccine-c627a5db5ad784335268ee6dbcb5e600\" target=\"_blank\" rel=\"noopener nofollow\"><u>researching<\/u><\/a> combined mRNA flu and COVID-19 shots, and those projects\u2019 status is unclear.<\/p>\n<p>The FDA also recently <a href=\"https:\/\/www.npr.org\/sections\/shots-health-news\/2025\/05\/20\/nx-s1-5405013\/fda-covid-vaccine-limits\" target=\"_blank\" rel=\"noopener nofollow\"><u>announced<\/u><\/a> that vaccine makers seeking approval for future COVID-19 vaccines, or boosters, would <a href=\"https:\/\/www.kff.org\/quick-take\/how-could-fdas-new-guidance-affect-covid-19-vaccination-coverage-in-the-u-s\/\" target=\"_blank\" rel=\"noopener nofollow\"><u>need to conduct<\/u><\/a> new randomized clinical trials of healthy populations.
